The Neonatal Center renders highly skilled specialized consultative, diagnostic and medical help to neonates and premature newborn children of Volyn Oblast and Lutsk.
The Neonatal Center structure:
-
neonate pathology – 20;
- premature newborn pathology – 20;
- neonate infection beds – 5;
- surgical beds for neonates – 5;
- neonatal intensive care unit – 15.
Main fields of activities:
- Consultative and diagnostic care of newborns and pre-term neonates of the Volyn Oblast and Lutsk including outpatient care (consultancy is done in Neonatology Room of Admission Department by a duty neonatologist every day from 9 a.m. till 6 p.m.).
- Treatment of newborns with postnatal adaptation disorders and neonatal period diseases.
- Development and implementation of new modern technologies in neonatal care, diagnosis and treatment as well as health improvement of newborns and children of early age preventive programs.
- Analysis of structure and causes of neonatal morbidity and mortality in Lutsk and rayons of the Volyn Oblast.
- Development and implementation of quality standards for examination and newborn treatment protocols.
- Follow-up of health condition and development of deep pre-term neonates and children with severe perinatal pathology which have been treated in the Neonatal Center.
- Education of mothers and members of their families concerning questions of physiology and pathology during neonatal period, feeding and care of newborns and prevention of infections.
- Constant medical education of the Neonatal Center’s medical staff.
- Organization of scientific conferences and seminars for physicians and nurses of the institution and oblast.
- Development of methodical materials on neonatal pathology and clinical examination of certain categories of neonates for neonatologists, pediatricians and parents (there have been developed algorithms of neonatal jaundice management, clinical examination of pre-term neonates at local medical institutions, recommendations for parents concerning peculiarities of pre-term neonates care and lactation management).

In the Neonatal Center
|
Indications for hospitalization of children to Neonatal Center:
- acute disorders of postnatal metabolic, circulatory and cardio-respiratory adaptation;
- severe perinatal damage of central nervous system;
- intrauterine and perinatal TORCH-infections;
- acquired infections in neonatal period (acute respiratory infections, enteral infections caused by pathogenic flora, pyoinflammatory diseases and sepsis);
- breathing disorders requiring respiratory therapy;
- intrauterine growth retardation;
- neonatal jaundice of different genesis which require additional examination and inpatient treatment;
- immunological diseases;
- endocrinopathies of neonatal period;
- birth defects requiring inpatient examination, treatment and care;
- surgical pathology of neonatal period which can be corrected (both congenital and acquired).
Contraindications to hospitalization in the Neonatal Center:
- birth defects incompatible with life;
- terminal conditions;
- enteric infections (salmonellosis, dysentery, escherichiosis) – child is referred to Infectious Diseases Hospital;
- highly contagious pediatric diseases (varicella, measles, rubella, epidemic parotiditis, pertussis) – child is referred to Infectious Diseases Hospital.
Hospitalization terms:
- planned hospitalization from delivery hospitals mainly on the 3rd–5th day of life with previous agreement with Chief of the Neonatal Center;
- urgent hospitalization – depending on a child’s condition, in case of stable hemodynamics and agreement with Chief of the Neonatal Center or duty doctor in the Neonatal Center;
- calling of transport resuscitation team is done through theAnesthesiology Department.
Procedure of children referral to the Neonatal Center:
- Newborns and pre-term neonates in the age of 0-28 days are referred to the Neonatal Center.
- During the first 3 days of life newborns requiring urgent care are hospitalized in case of stable hemodynamics and if transport conditions are satisfactory.
- If care is not urgent, children are referred to the Neonatal Center on the 3rd – 5th day of life.
- is necessary to have an extract from Prenatal Record, Delivery History and Newborn Record.

Transporting a newborn
Diagnosis and Treatment in the Neonatal Center:
- blood tests (general analysis, biochemical, medical and genetic examinations, blood gases, acid-base condition, immune-enzyme analysis, serological reactions);
- urine tests;
- stool tests;
- cerebrospinal fluid tests;
- bacteriological and virus examinations;
- cytological examinations;
- X-ray examinations;
- electrocardiography (ECG);
- ultrasound examinations;
- computer tomography (CT) and magnetic resonance scan (MR).
Methods used in newborn care:
- drug administration (intravenous, intramuscular, intradermal, oral and local injections);
- physiotherapeutic procedures;
- hygienic and therapeutic baths;
- weightlessness imitation (dry immersion;
- aromatherapy;
- massage, exercise therapy;
- position therapy;
- phototherapy;
- "Kangaroo" method;
- alternative feeding methods when mother is separated from her child;
- family oriented neonatological care (participation of mother and other family members in treatment and care of a newborn).
Modern technologies of neonatal care implemented in the Neonatal Center:
- "minimal touch" principle (avoiding external stimulus influence on small premature and severely sick infants);
- antireflux position, position therapy;
- skin, umbilical remains and umbilical wound care according to WHO and UNICEF recommendations;
- warm protection ("warm chain" maintenance);
- adequate newborn feeding (breast, alternative and therapeutic, if indicated, feeding).
The Neonatal Center is preparing to gain the status of "Hospital Benevolent to a Child".
11 principles of breast feeding are implemented in the Center. Breast Feeding Policy Trainings with the staff are provided for 18 hours annually. "Young Mother's School" works in the Neonatal Center where breast feeding trainings are provided to mothers and their family members. The Neonatal Center staff attended breast feeding trainings and received certificates of the Regional Center for Lactation and Breast Feeding Support (Lviv). The kangaroo method
Protective environment is introduced in the Neonatal Center:
- decreasing light and sound stimulants;
- warm protection;
- mother and child stay together;
- mother’s participation in child’s care in Resuscitation Department;
- breast feeding on infant’s request;
- usage of home cloths and toys for child;
- qualitative care of newborns who stay without mothers by medical staff;
- training and assistance to mothers and their family members regarding newborn care, breast feeding and lactation;
- favorable communication with mothers and other relatives of a newborn, giving full information about their child’s condition, necessary diagnostic and therapeutic procedures;
- favorable communication among medical staff, keeping medical ethics and deontology principles.
Structural subdivision of the Neonatal Center is Department of Anesthesiology for Newborns and Premature Neonates, which includes 15 beds.
The Department’s tasks:
- providing highly specialized anesthesiology care to newborns and premature neonates of Volyn Oblast and Lutsk;
- recovery, correction and support of essential organs and systems functions damaged by disease, trauma, surgical intervention or other reasons;
- preparation and conduction of general anesthesia during surgical intervention, diagnostic and therapeutic procedures in newborns;
- 24-hour therapeutic and consultative care to newborns in the Neonatal Center of the Volyn Regional Children’s Territorial Medical Center, obstetrical and children’s hospitals in oblast;
- organizational and methodical work, further training of neonatologists on main principles of resuscitation and intensive care of newborns;
- implementation of new medical technologies and intensive care methods.
Indications to hospitalization in the Department of Anesthesiology for Newborns and Premature Neonates:

In the Anesthesiology Department
|
- Full-term and pre-term babies with age of up to 28 days of life and pre-term neonates with corrected age of 40 weeks are hospitalized to the department.
- Children with respiratory insufficiency of ˛˛-˛˛˛ stage.
- Children with acute cardio-vascular insufficiency.
- Children with shock of different etiology.
- Children with acute renal insufficiency.
- Children with convulsion syndrome.
- Children with severe water-salt disorders.
- Children with severe hyperbilirubinemia.
- Newborns with severe asphyxia, birth injury with multiple organ failure.
- Newborns requiring full parenteral feeding.
- Newborns with surgical pathology followed by severe disorders of essential systems.
- It is not recommended to transfer to the Department from other facilities newborns in terminal phase of any disease and children with birth defects which cannot be corrected at the moment.
The following technologies are used in the Department’s work:
- artificial lung ventilation including oscillatory;
- substitute surfactant therapy;
- infusion therapy and parenteral feeding with central and peripheral venous access;
- different forms of oxygen therapy;
- intensive phototherapy;
- pre-surgical preparation, anesthetic provision of surgical interventions, post-surgical care;
- micro methods of blood gases, electrolytes and glucose identification;
- X-ray and ultrasound examination;
- "warm chain" principles maintenance;
- participation of mothers and their family members in feeding and care of children in the department.
The department is equipped with modern respiratory devices, incubators including incubators for newborns with extremely low birth weight, infusion feeders, monitors of vital functions and other equipment.
Transportation of newborns from other oblast hospitals to the Department of Anesthesiology for Newborns and Premature Neonates is done by the mobile neonatal team of urgent care and intensive therapy in a specially equipped resuscitation car.
The neonatal team is created in order to provide highly specialized anesthesiology care to children in obstetrical, children's and anesthesiology facilities in oblast and transfer them to the Neonatal Center.
The team’s tasks:
- 24-hour consultative assistance to newborns in different oblast hospitals can be provided by visiting them or by phone.
- Preparation of newborns to transportation (activities to recover, correct and support the damaged essential functions).
- Transportation of newborns and premature neonates to the Neonatal Center; intensive therapy during transportation.
